Overview

NaphCare is hiring a Social Services Case Manager to join our team of medical professionals in North Brunswick, NJ, where we manage all aspects of healthcare for the individuals who are currently incarcerated in the Middlesex County Jail.

 

Pay rates vary based on shift, site needs, and experience and range from $24.70 to $26.00. Shift and hours vary based on coverage needs and can be discussed upon interview.

 

As a Social Services Case Manager, you will work alongside correctional officers and other medical professionals to provide care to the individuals in residence. Working within a controlled environment, where security is top priority and the days are never dull, you will have various duties to keep you engaged including performing physical assessments, coordinating social services activities for inmates, attend classification meetings, refer patients to other clinicians and providers and participate in discharge planning activities.

Responsibilities

  • Use knowledge and expertise to ensure that processes and systems are followed appropriately according to policies and procedures, company performance and accreditation standards.
  • Work cohesively with the Sr. Case Manager, Social Services Director, HSA, Physicians, Mental Health Director and psychiatric provider(s) in the coordination of the transitions in levels of care or release planning.
  • Use all functions and applications of TechCare relevant to the position with no supervision and assist with staff training as requested
  • Communicate effectively with clients, peers and employees at all levels within the organization. Maintain clear lines of communication throughout issue-resolution processes.
  • Demonstrate personal performance based on professional practice standards, relevant statutes, rules and regulations and organizational criteria.
  • Demonstrate professionalism by using objectivity, maintaining self-control when under pressure, separating professional from personal issues, displaying emotional maturity, maintaining a high degree of integrity and promoting a positive work environment
  • Relay critical information to the next level of management that may have a negative impact on client and/or vendor relations or inmate care.
  • Demonstrate the knowledge and skills necessary to refer for appropriate care, based on the physical, psychosocial, educational, safety and related criteria appropriate to the age of the inmate served.
  • Participate in monthly team meetings, all educational and training activities required by healthcare staff and security personnel.
  • Evaluate inmates in a variety of settings including, but not limited to, initial intake area, medical unit and housing units.
  • Coordinate with attorneys and Court regarding inmate’s program release date.
  • Assist inmates in contacting Courts and attorneys to resolve issues and be informed about future court proceedings.
  • Assist in discharge planning to include in-patient programming, identification of a support network, referrals to Re-entry agencies, mental health agencies, substance abuse agencies, etc., based on the needs of the inmate.
  • Coordinate educational and psychoeducational programming for inmates.
  • Ensure that inmates with disabilities have all the appropriate services provided for them in accordance with the Americans with Disabilities Act.

Qualifications

Experience / Requirements / Qualifications

  • Bachelor’s degree in a human service or mental health field
  • Valid CPR card

 

Physical Capabilities:

Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.
